We are looking for a compassionate and proactive Support Worker to help vulnerable individuals take steps toward independence and stability. Youandrsquo;ll assess client needs, deliver tailored support plans, and work closely with partners such as the National Probation Service and Police. Key Responsibilities
- Build positive relationships with external agencies, including criminal justice services
- Complete needs and risk assessments to create individual support plans
- Advise clients to help them access services and accommodation
- Ensure new clients understand their rights, responsibilities, and licence conditions
- Contribute to record-keeping and reporting for partners and funders
- Valid UK driving licence and access to your own vehicle (business insurance required for travel)
- Willingness to participate in an out-of-hours on-call rota
- Experience supporting vulnerable adults and/or people with complex needs and challenging behaviours
