Salary: andpound;25,685 per year
Hours: 37.5 hours per week

About the Role
We are looking for a compassionate and proactive Support Worker to help vulnerable individuals take steps toward independence and stability. Youandrsquo;ll assess client needs, deliver tailored support plans, and work closely with partners such as the National Probation Service and Police. This role combines practical support, advocacy, and multi-agency collaboration to make a real difference in peopleandrsquo;s lives.

Key Responsibilities
  • Build positive relationships with external agencies, including criminal justice services
  • Complete needs and risk assessments to create individual support plans
  • Advise clients to help them access services and accommodation
  • Ensure new clients understand their rights, responsibilities, and licence conditions
  • Contribute to record-keeping and reporting for partners and funders
Requirements
  • Valid UK driving licence and access to your own vehicle (business insurance required for travel)
  • Willingness to participate in an out-of-hours on-call rota
  • Experience supporting vulnerable adults and/or people with complex needs and challenging behaviours
