Leaving Care Worker
A Local Authority is seeking a Personal Advisor to support young people leaving care and transitioning into independent adulthood.
Key Responsibilities:
- Act as a named advisor for care leavers aged 18+
- Support young people aged 16-18 alongside Social Workers to prepare for independence
- Deliver one-to-one and group interventions to develop life skills and improve education and employment outcomes
- Complete Needs Assessments and Pathway Plans
- Provide advice on housing, health, education, and employment
- Work with multi-agency partners to coordinate support
- Undertake visits, maintain accurate records, and manage caseloads
Requirements:
- Level 3 qualification in a relevant field or substantial equivalent experience
- Experience working with vulnerable young people or care leavers
- Knowledge of safeguarding and relevant legislation
- Strong communication and organisational skills
- Ability to work independently and manage competing priorities
Additional:
- Flexible working, including occasional evenings and weekends
- Ability to travel as part of the role
