Leaving Care Worker

A Local Authority is seeking a Personal Advisor to support young people leaving care and transitioning into independent adulthood.

Key Responsibilities:

  • Act as a named advisor for care leavers aged 18+
  • Support young people aged 16-18 alongside Social Workers to prepare for independence
  • Deliver one-to-one and group interventions to develop life skills and improve education and employment outcomes
  • Complete Needs Assessments and Pathway Plans
  • Provide advice on housing, health, education, and employment
  • Work with multi-agency partners to coordinate support
  • Undertake visits, maintain accurate records, and manage caseloads

Requirements:

  • Level 3 qualification in a relevant field or substantial equivalent experience
  • Experience working with vulnerable young people or care leavers
  • Knowledge of safeguarding and relevant legislation
  • Strong communication and organisational skills
  • Ability to work independently and manage competing priorities

Additional:

  • Flexible working, including occasional evenings and weekends
  • Ability to travel as part of the role
